What Is The Assistive Technology and Home Modifications (AT-HM) Scheme?

The AT-HM Scheme is a new funding model designed to streamline how older people access:

  • Assistive Technology (AT) - devices and equipment that support day-to-day independence
  • Home Modifications (HM) - environmental changes that make the home safer and easier to navigate

The AT-HM Scheme may fund:

Assistive Technology

  • Adaptive kitchen and bathroom equipment

Home Modifications

  • Grab rails and handrails
  • Threshold ramps and stair lifts
  • Bathroom adjustments (e.g. handheld showers, accessible fittings)
  • Door widening and step alterations
  • Non-slip flooring and safety enhancements

Who's Eligible?

The AT-HM Scheme is available to individuals who:

  • Are aged 65 years or over (or over 50 years for Aboriginal or Torres Strait Islander people)
  • Have been assessed and classified under the Support at Home Program
  • Have identified functional limitations that can be addressed through AT or home modifications

You'll need to go through a My Aged Care assessment to access this scheme.
