The End-Of-Life Pathway offers short-term, intensive support for older Australians approaching the final stage of life. It focuses on:
- Prioritising comfort and quality of life
- Providing personalised, home-based care plans
- Supporting carers, family, and loved ones
- Ensuring access to clinical and allied health services for symptom management
- Respecting the individual's wishes to remain at home, where possible
This pathway ensures that those needing end-of-life care receive timely, coordinated support, avoiding unnecessary hospital or residential care admissions wherever possible.

Who's Eligible?
The AT-HM Scheme is available to individuals who:
- Are aged 65 years or over (or over 50 years for Aboriginal or Torres Strait Islander people)
- Are living at home and have a life-limiting condition
- Are assessed by My Aged Care as requiring end-of-life support
