What Is The Restorative Care Pathway
The Restorative Care Pathway offers a tailored care episode for people who are:
- Recovering from illness, injury, or hospitalisation
- Experiencing early signs of functional decline
- Adjusting to a new or changing age-related condition
- Looking to avoid or delay entry into long-term or higher-level care
This pathway focuses on restoring and maintaining function, rather than simply managing decline. Through a team-based approach, participants receive weekly (or more frequent) assessments and support designed to help them achieve meaningful, personal goals.

Restorative care is delivered over a short-term period of up to 12 weeks, depending on your needs and goals. The intensity of services will vary, but frequent check-ins and flexible care adjustments are part of the process.

- Are recovering from illness, injury, or a hospital stay
- Have been newly diagnosed with an age-related condition
- Want to build skills and confidence to live independently
Your eligibility will be determined through My Aged Care , and funding will come from your Restorative Care budget under Support at Home.
